Use the following forms to notify CAF Headquarters of your events, access the general Hold Harmless From, or submit an incident report.

Education Program Hold Harmless 

Send this form to parents, teachers, and other adults responsible for the students visiting your location or participating in your programs. Custom forms can be designed on request.

Incident Report

Use this form to notify CAF Headquarters of any incidents that take place during one of your education programs.
